Validation of the Inflammatory Response Scale in acute appendicitis

Abstract

The purpose of this study was to determine the validity of the acute appendicitis inflammatory response scale (AIR) in patients aged 16 to 35 years at the Vicente Corral Moscoso Hospital, Ecuador. An epidemiological, analytical, cross-sectional study was carried out by applying a diagnostic test designed on the AIR scale correlated with histopathological findings; for which 177 patients were considered. The data is analyzed with SPSS (24.0), using measures of central tendency and dispersion, Sensitivity (S), Specificity (E), positive and negative predictive value (PPV and NPV), positive and negative likelihood ratio, ROC curve, the average age of the participants was 25.11 (SD ± 5.3). 10.58% of the patients who had an AIR score ≥ 8 were positive of acute appendicitis on histopathological analysis. AIR S ≥ 8 was 10%, E = 100%, PPV = 100%, NPV = 18.63%. The efficiency of the AIR test ≥ 8 was = 25.99%. The probability of having a negative result (AIR < 8) was 89% (negative likelihood ratio). The validity of the AIR test ≥ 8 has a low probability as a predictor of acute appendicitis.
