Necrotizing fasciitis in a public hospital in Ecuador: Case report

Abstract

Necrotizing fasciitis is a foreign bacterial infection that spreads rapidly through the organism and can cause complications and death. It is classified according to the microorganism into three types: multimicrobial infections caused by aerobic and anaerobic bacteria, by group A Streptococcus and Staphylococcus aureus and by Vibrio vulnificus. The aim of this article is to provide an overview of the management of this pathology in the context of public health in Ecuador. We present the case of a 55-year-old patient, with no personal pathological or surgical history. The physical examination showed edema on the left lateral side, after the CT scan a mass was observed in the left hemithorax region, with involvement of subcutaneous cellular tissue of approximately 10x5cm, in addition complementary examinations showed a leukocytosis of 23350. Necrotizing fasciitis is a surgical emergency and inadequate or late treatment is associated with high mortality, the debrided area should be constantly monitored to verify that the process has not spread and assess the possibility of another debridement. Treatment involves, in addition to immediate fasciotomy, the initiation of antibiotics with a broad-spectrum coverage and are adapted according to the results of culture and renal function.
