Dextrotransposition of the great arteries surgical correction

Abstract

Dextrotransposition or transposition of the great arteries is a congenital pathology that produces an anatomical defect in relation to the arteries that transport blood out of the heart, these are inverted as well as the ventriculo-arterial connection, where the aorta artery is connected almost in its entirety to the right ventricle, in the same way the pulmonary artery is linked to the left ventricle, discovering a functional anomaly to the organ and its functioning, becoming fatal without surgical treatment. This is a clinical case of a full-term male newborn, with fourth-degree prenatal history in consanguinity. Apgar 5-7 with need for advanced CPR. He was admitted to the Neonatal Unit diagnosed with transposition of the great arteries. He underwent surgery using the Janete technique. During the surgical act, the temperature was lowered to 32 OC and cardiac arrest was completed accompanied by extracorporeal circulation for 2 hours and 25 minutes. Immediate postoperative requiring vasopressors, which were gradually reduced until they were discontinued and the patient was successfully extubated. The transposition of the great vessels turns out to be a rare disease, but fatal as it does not resolve the pathology; surgery being the best choice when making therapeutic decisions. Janete's surgery represents the best option when seeking to correct defective vessels.
